First Visit - Consultation

Each initial appointment consists of a consultation explaining diagnosis and treatment options. Simple procedures can sometimes be performed the same day as the consultation. However, a complex medical history and most surgical procedures will require an evaluation and a second appointment to provide treatment on another day.

The following information should be provided at the time of your initial consultation:

  • A referral form from your dentist and any x-rays if applicable (this may be sent to us by your dentist).
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the information required to file your insurance.
  • Please take advantage of our online Patient Information and Registration Form and bring the completed form to your appointment.
  • Fees for the first appointment are expected in full.

IMPORTANT: All patients under the age of 18 years of age must be accompanied to the consultation visit by a parent or guardian.

A preoperative consultation and examination with complete medical history is mandatory for patients undergoing intravenous (IV) anesthesia for surgery. For those having IV anesthesia, the following two requirements must be followed: Nothing to eat or drink six hours prior to surgery. An adult must be with you to drive you home.

When making an appointment, please alert the staff if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. We will make every effort to use these x-rays. Sometimes previous x-rays cannot be used because they are too old or may not adequately show the areas of interest. If additional films are necessary, they can be taken at our facility.
